Where on the server do I install this licence?
Nowhere. SQL CALs (like Standard Windows Server CALs) are purely legal proof of licensing. You must keep the invoice and the licence documentation as proof. There is no technical installation or activation on the server.
How does this differ from a SQL Device CAL?
A User CAL is tied to a user, who can then access the server from any number of devices. A Device CAL is tied to a device, which can then be used by any number of users. Rule of thumb: if employees have several devices, the User CAL is better.
Do these CALs also work if I have bought the core-based licence for SQL Server?
No. If you have licensed SQL Server per core, you need no CALs. CALs only apply to the ‘Server + CAL’ licensing model.
Can I also use this CAL to access a Windows Server file share?
No. SQL Server CALs are entirely separate and only apply to accessing SQL Server. To access Windows Server services (files, printing), you need separate Windows Server CALs.
